Lost MSB-8 Certificate Replacement Request (Expedited)

This form is for requesting an expedited re-issue of your lost MSB-8 certificate. Be sure and properly fill out where you want your new certificate sent - especially your email address. Once you submit the required information you will be redirected to our secure payment processing site. There you will be charged a $45 processing fee. After payment is received you should expect to receive an emailed copy of your new certificate in 1-2 days, along with the mailed copy in 5-7 days, if we are able to find your information.
